Forms For Power Of Attorney

Q: Where do I take Power of attorney forms after they're notarized? On the TN gov website, I found Power of Attorney papers I need to fill out (for my mother's medical issues). I know I have to have a Notary public stamp & sign it...does anyone know 1) How much Notary Pubilc's charge to do this & 2) What would I do with the paperwork after that's done? Would I need to file it to the courthouse or anything?

A: Was your mother competent when she filled this out, or was this sought out through a court order or through the State Attorney's office (or in Ontario, Public Guardian and Trustee's Office)? If there is no agreement, a court order is sought. The person who is the Attorney for Health Care receives a copy, as well as that person's primary health care provider and if it involves financial decisions as well, it would also be copied to the person's bank and other financial services providers. In some states, you may have to register it to the equivalent of the Public Guardian and Trustee.
