Power Of Attorney ,info???

Q: I live in VA and want to establish a Power of Attorney (POA). My STBX wants to give me POA limited to depositing/cashing/using a check from a certain source. The financial institution involved says that they do not have the forms to do this and that we had to create them with or without the help of an attorney. Once we provide the financial institution with the POA, then I have to sign an agreement that makes me responsible rather than the institution-- seems ok. Is anyone aware of a template of such a POA for VA. If so please advise.

Had to go through a lawyer for the power of attorney to keep the bank happy.
