The SQ006 crash have some how revealed the safety management of the Taiwan
authority screwed up in case of incident.
Main factor: The pilot and it 2 co-pilots have drove the plane into the
wrong runway. No denying! But so far report from the press (not only
Singapore media) including CTN, the Taiwan authority direct the blamed
entirely to the pilots and not the airport authority. A simple words by the
chief said that they would "examined further to see whether the controller
officials are also negilance". How long it would take?
Have they (Taiwan authority) analyse contributing factors? Let me briefly
list out the factors:
1) Weather on that night. (THE MOST IMPT CONTRIBUTING FACTOR)
2) The so-called China Airline pilots that claim to witness the SQ plane was
in the wrong runway. So with so many days have passed, no a single words
that these 2 pilots have be asked for a formal investigation?
3) What the duty personnel are doing in the control tower?
4) Was the lights along the close runway on?
5) A runway that have been closed for so many year, shouldn't it not call a
runway anymore?
6) Proper closure /safety procedures done on the so-call under-repair
runway".
7) Conduct & personal history on the 3 SQ pilots. (To be investigate by SIA
too) Any sucidal thought? Suffering any depression?
8) Proper safety management procedures carried out by the airport authority.
E.g a risk assessment form correctly filled up by the pilot and the airport
official.
No one want this incident to happen! However, when the investigation is
carried out, the committee have to be logical and unbaised on which ever
sides so as to give the dead ones a fair trial. I bet you too don't want to
die without knowing the cause of your departure from the earth.
Next, Safety Management in Singapore. I'm not be boasting of my country
organization or image. But I felt our government have done well in term of
safety management to prevent any crises to happen.
1) The recent HFM widespread incident, only 1-2 days when 2 kids died in
related to the dieases. A multi-ministrial commitee was formed to
investigate the matter. Preventive measures like a more than 1 weeks closure
of all child-care centres. The civil service have also taken the lead to
ensure that parents involved would have enough time off to take care of
their kids. (At that time of the incident, I was in Taiwan for an SAF
oversea exercise.) Knowing the cases though hp and the ST available in camp.
2) MINDEF and SAF safety management in the training of our troops. Having
about to complete my National Service in 2 months times. During this period,
I'm really impressed of the SAF
A: - The weather is deem to be within limits. What is wrong with the pilot's decision to take off ?
